Preservé by Motiva places your implant above the chest muscle through a small incision in the breast crease. Instead of cutting tissue to make room, we gently separate and stretch what is already there. Dr. Tal offers Preservé to carefully selected patients who want natural-looking volume and a faster return to daily life. Candidacy depends on your anatomy, your skin quality, and how much volume you want. Results vary from patient to patient.

Not every breast augmentation candidate wants to go through a muscle-cutting procedure to get there. Preservé was designed for exactly that patient. Dr. Tal T. Roudner, MD, FACS offers this tissue-preserving technique. It gives you fuller, natural-looking volume without the trade-offs of a traditional Breast Augmentation. What Is Preservé Breast Augmentation?

Preservé by Motiva Breast Augmentation is a tissue-preserving approach utilizing Motiva’s SmoothSilk Ergonomix implants, that leaves your natural anatomy in place. . The pectoralis muscle stays intact. The ligaments, nerves, and blood vessels that support your breast stay intact. We do not cut through muscle or burn a path for the implant.

To make room, we gently part the tissue with a blunt instrument. A temporary sizing balloon then shapes the pocket before your implant goes in. A protective sleeve keeps the implant from touching your skin on the way in.

Preservé is designed for first-time Breast Augmentation, including first-time augmentation combined with a lift in appropriate cases. We will walk you through what to expect for your anatomy at your consultation.

How Does Preservé Breast Augmentation Work?

The procedure follows three steps through a single small incision hidden in the fold beneath the breast.

  • Creating space. We gently part the tissue with a blunt instrument, moving around it rather than cutting through it.
  • Shaping the pocket. A sizing balloon fills to your planned volume, and the tissue stretches to fit around it.
  • Placing your implant. The balloon comes out, and your implant goes in through a protective, no-touch sleeve.

No-touch insertion is linked to a lower capsular contracture risk and often a shorter incision than manual placement. We have used the Keller Funnel no-touch technique in our Breast Augmentation practice for years.

How Is Preservé Different From Traditional Breast Augmentation?

Traditional Breast Augmentation usually means general anesthesia, an incision of roughly three to five centimeters, cautery to build the pocket, and placement under or partially under the chest muscle. It is safe, flexible, and supported by decades of data. It also handles large implants, significant sagging, and revision cases that Preservé cannot.

Preservé trades that flexibility for a gentler experience: sedation instead of general anesthesia, a smaller incision, no muscle release (which means no animation deformity when you flex), and a shorter recovery.

Preservé is not a better operation. It is a different one, better for a specific patient. We will tell you plainly if you are that patient.

Am I a Good Candidate for Preservé Breast Augmentation?

Preservé suits patients who want a modest, natural-looking increase and who value getting back to an active routine quickly. You may be a strong candidate if you:

  1. Are seeking your first Breast Augmentation
  2. Have firm skin with little or no sagging
  3. Have enough natural tissue to cover an implant
  4. Want a subtle change rather than a dramatic one

Preservé is likely not the right choice if you:

  1. Want a large increase in volume
  2. Have significant sagging that calls for a Breast Lift
  3. Already have implants and need Breast Implant Revision

Prior breast surgery has already changed the internal anatomy that Preservé is designed to preserve.

Two firm requirements apply. Silicone implants are FDA-labeled for augmentation only in patients who are at least 22 years old. And if you smoke, you must stop 4 weeks before surgery and remain smoke-free for 3 weeks afterward.

What Is Recovery Like After Preservé Breast Augmentation?

Many patients are up and walking the same day, and many manage discomfort with over-the-counter medication. Light daily activity typically resumes within a few days. Full exercise is commonly cleared around two weeks, considerably faster than the four to six weeks often advised after submuscular.

Swelling settles over several weeks, and the implants continue to soften and drop into their final position over the following months. Some tightness and occasional shooting sensations are normal in the early weeks. Recovery is not the same for everyone, and we will give you a timeline built around your case rather than an average.

Will Preservé Affect Nipple Sensation or Breastfeeding?

Sensation changes are less likely with Preservé than with techniques that divide tissue. That is because the nerve pathways to the nipple are left undisturbed.

That said, sensation changes are a recognized complication of any breast implant surgery, and they can be temporary or permanent. Because glandular tissue and ducts are preserved, many patients retain the ability to breastfeed, though no surgeon can promise this.

 

What Risks Should I Understand Before Choosing Preservé?

Preservé is still surgery, and breast implants carry risks regardless of technique. Complications such as capsular contracture, rupture, malposition, infection, and asymmetry can occur, and breast implants are not lifetime devices.

One limitation deserves naming. Because the pocket is created without direct visualization, we rely on measurement and feel rather than sight. That places real weight on surgical judgment and experience.
